MOVING TO THE M-S DISTRICT?

• Call Mahomet-Seymour High School (217-586-4962) to set up an appointment to meet with your counselor.
• Arrive to your scheduled appointment on time with the following items:
  • Proof of residency (copy of a lease or sales contract)
  • Copy of student's birth certificate
  • Copy of updated physical/health/immunization records (Illinois law requires all incoming 9th graders to have a physical. If you are transferring from outside Illinois, a physical will be required to complete your enrollment).
  • Illinois State Transfer Form (you can get this form from your previous school)
  • Copy of divorce decree (if applicable) showing proof of legal guardianship
  • Copy of grades/transcript from previous school (this is not required to complete registration but would be an enormous help in planning your schedule)

At your scheduled appointment, you will...

...meet your counselor
...take a brief tour of the high school
...receive a tentative class schedule, locker assignment
...learn about general school policies and practices
